• In collaboration with other registered nurses on the team; develop, revise, maintain, and supervise medical treatments and medication policies and procedures including transcribing, administering, evaluating and recording psychotropic medications prescribed by the team psychiatrist; evaluate and chart psychotropic medication effectiveness, complications, and side effects; and arrange for required lab work according to protocol.
• In collaboration with other nurses on the team, organize and manage the system of getting medication to clients and integrating medication administration tightly into clients’ individual treatment plans.
• In collaboration with other nurses on the team, manage pharmaceuticals and medical supplies.
• Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment.

• Organize and facilitate regularly scheduled treatment team meetings to facilitate person-centered planning, revisions and updates. Include formal and informal supports as appropriate.
• Ensure interventions are based on recommendations from assessment activities, are supported by documented medical necessity, and are implemented as documented in the PCP.
• Provide individual and or group therapy
• Coordinate discharge planning for clients in hospital, residential services, or other levels of care.
• Coordinate and assist clients with community re-entry following hospitalization, residential services, and other levels of care.
• Complete clinical documentation for all services and client related activities in a timely manner
• Perform shift management coordination with other ACT team members according to established policies and procedures.
• Provide on-call crisis intervention covering nighttime hours and backup evening and weekend staff.
• Participate in daily staff organizational meetings and treatment planning review meetings as required

Qualification/Licenses

• Must be licensed to practice in the District of Columbia as a Registered Nurse
• Must have documented experience providing treatment to adults with severe and persistent mental illness
• Must possess a valid driver’s license with a safe driving record
• Must be able and willing to work flexible schedules to include working early mornings, evenings, and weekends per consumer need
